Respite Care

For ALTCS members, respite care is typically obtained through the member's ALTCS case manager and health plan. Respite care is a covered Home and Community-Based Service designed to provide temporary relief for unpaid caregivers.  ATLCS policy usually allows for 600 hours of respite care per benefit year (October1-September 30).

Medical Caretaker

When you have a family member who is covered by ALTCS services, you can choose home based services which means you elect them to receive care at home, not a facility.  Family members can be hired through an ALTCS-contracted agency or through member-directed programs such as Agency with Choice (AWC) or Self-Directed Attendant Care (SDAC). Caregiver will need to complete onboarding, background check, training, CPR/First Aid, and any other agency requirements.

• Parent/Guardian files a petition for appointment of guardianship of an adult, documents can be picked up at a court house or printed from https://superiorcourt.maricopa.gov/llrc/prob_group_2/

• Complete documents adding patient’s information and then drop off at Arbor office for clinician to complete medical report.  Leave case number blank, this will be assigned by the Court Clerk.

• Follow steps 1-4 in packet 2 once you have completed Medical Report.

• Must meet federal definition of permanently disabled, live in household with little or no income.

• Have never been gainfully employed.

• Acquired their disability prior to age 18.

IEP - Individualized Education Plan

Provided under the federal law known as the Individuals with Disabilities Education Act (IDEA). It is designed for students who have a qualifying disability that adversely affects educational performance and who require specialized instruction. Qualifying disabilities may include Autism , Intellectual disability, Specific learning disabilities (e.g., dyslexia), Speech/language impairment, Emotional disability and Other health impairments (including some students with ADHD).

504

Available through Section 504 of the Rehabilitation Act and provides accommodations that allow a student with a disability to access education but does not include specialized instruction.  Accommodations may include : extended test time, preferential seating, modified assignments, health-related accommodations and behavioral supports.

1. Process to obtain IEP or 504: Submit a written request to the school.

   • Send a letter or email to the principal, special education coordinator, school psychologist, or counselor.

   • State that you are requesting an evaluation for special education services (IEP) and/or Section 504 accommodations.

2. Describe your concerns.
   Include:

   • Academic difficulties, behavioral concerns, medical diagnoses (if applicable), social or communication challenges, any outside evaluations or recommendations

3. Provide supporting documentation.
   Examples:

   • Medical records, psychological evaluations, therapy reports, teacher concerns, report cards and test results

4. The school reviews the request.

   • For an IEP evaluation, the school typically obtains parental consent before conducting assessments.

   • For a 504 Plan, the school reviews available information and determines eligibility.

5. Eligibility meeting.
   The school team reviews evaluation results and determines:

   • Whether the student has a qualifying disability.

   • Whether an IEP or 504 Plan is appropriate.
